The Determining the Role of Endothelium and Extracellular Matrix in Thoracic Aortic Aneurysm

The goal of this research was to look at the pathophysiology of thoracic aortic aneurysms from a morphological standpoint. Thoracic aneurysms are more prevalent in the ascending aorta and arch, accounting for 60% of all cases, and 40% in the descending aorta, however the etiological causes are different in each area. While atherosclerosis, bicuspid aortic valve, familial thoracic aortic aneurysm syndrome, Turner syndrome, Marfan syndrome, aortic arteritis in Takayasu’s arteritis, trauma, and dissection can all lead to thoracic aortic aneurysms, the structural changes to the basement membrane and the exact causes of the mechanism are unknown. Between 2007 and 2017, a total of 12 patients with thoracic aortic aneurysms who were contraindicated to interventional treatment had surgery. Multi-detector row echocardiography and transesophageal echocardiography All patients had CT scans with three-dimensional (3D) reformation in 3 mm slices. The endothelium is partially desquamated, the collagen fibres and basal membrane are significantly altered, and repair attempts in the form of endothelial islets and deep fibre rips, as well as cancellation and destruction of the helical structure of collagen fibres, are made. In thoracic aortic aneurysms, we identified significant structural changes in collagen fibres, endothelium, and basement membrane, which are similar to acquired aortic valve degeneration.
